Steps for Participation in the Goodie Bag Program
1. Download: the Goodie Bag Form and Avery Label template files.
*Requires free Adobe Acrobat Reader to view and print.
2. Call : Matt Nolin to participate in the goodie bag program.
3. Payment: Arrange to make payment with The Sample Guy.
4. Shipping: You will receive a shipment authorization code and the shipping address when you sign up for the goodie bag program.
5. Deadline: Inserts must be shipped to "The Sample Guy" goodie bag stuffing warehouse in time to arrive between October 20th and October 27th, 2008 .
